OVERVIEW

Evercare is a service design system that bridges hospitals, patients, families, and community support in cardiovascular hospice care. It integrates a digital platform and community services to provide health monitoring, education, and emotional support, fostering a more connected and compassionate care network.

BACKGROUND

In August 2018, a family member suffered a sudden cerebral hemorrhage and passed away after six months of home care, during which both they and my family endured immense physical and emotional pain.


Similarly, in January 2023, another family member passed away after seven years of living with the debilitating effects of a cerebral infarction. Unable to move or speak, they faced profound frustration, while our family devoted ourselves tirelessly to their care, navigating countless emotional and logistical challenges.

Participants: 4 patients (aged 49-93) and 4 family members.

Focus: Cultural attitudes toward end-of-life care, caregiving challenges, and support needs.

Hand-code the interviews to turn qualitative data into quantitative data. Each interview was transcribed and codes were derived from those interviews. The codes were later combined to generate four main themes.

The interviews revealed the complex challenges participants faced with end-of-life issues, exposing critical gaps between China’s hospice healthcare system and home care for chronic illnesses, highlighting systemic service shortcomings.

THE SUMMARY

To understand the interactions between stakeholders and define a structured care model, a system map was created to visualize how services, organizations, and individuals connect within the solution. This mapping clarifies the flow of resources, responsibilities, and communication between medical institutions, community healthcare, caregivers, and support organizations.

To visualize and better understand the systems map, I created several storyboards that illustrate the interactions between the patient, family, and community healthcare.

Service blueprint

This service blueprint provides a step-by-step guide for supporting patients with heart and brain-related diseases, as well as their families. It uses the Care App to help patients monitor their health, learn about their condition, and access psychological support. The plan also involves hospitals, community healthcare, and NGOs to offer medical care, health education, and emotional support. From diagnosis to recovery—and even after a loss—this blueprint ensures patients and families receive the care and resources they need at every stage.

To bring the idea to life, I started with a low-fidelity prototype—simple, hand-drawn screens to explore how users might navigate the platform. I focused on key moments: checking blood pressure, finding palliative care knowledge, and joining community events.

After Thoughts

This project allowed me to deeply reflect on the emotional and practical needs of patients and families during home-based end-of-life care. From initial research to final hi-fi design, I learned the importance of empathy, clear communication, and accessible support systems. While the journey had challenges—like balancing sensitivity with functionality—it also reinforced the value of design in creating meaningful change.
